Please help me clear up the cementation protocol.
Sealed prep with Simplicity 1 and 2 after impression and temp fabrication.
temp onlay was cemented with Patterson temp cement NE.
onlay seat appt: remove temp , clean temp cement with Simplicity 1, apply one fresh coat of Simplicity 2 (no light cure), Anchor on onlay (Interface and Simplicity 2 light cure then Simplicity 2 air dried), clean off excess.

On the tooth

I'd recommend you re-apply Simplicity as normal, then add 1 more coat of Sim 2, not light-activated.

On average, what is chance that patient will need to be anesthetized for this cementation protocol?

For re-bonding, I will usually anesthetize. For simple cementation I will not.
