Is this a good idea? What protocol should I follow when delivering an emax crown?
I just delivered an emax implant crown, prepared the crown with interface/sim1/sim2, and it popped off after cleanup (about 20 minutes post-delivery-- I did a prophy while it was setting). Isolation was great, but I did not initiate the anchor with a light source as I figured it would be stronger by self-curing... What should I have done differently?

I don't ever use permanent cement on my implants. With the minimal taper, you almost don't need any. If anything should happen to the screw (loosen for example) I can always retrieve the crown without destroying it. I think premier makes an implant cement. If I was going to use permanent cement, I'd use a luting cement like rely x. _________________ Rich (cool kid) Rosenblatt

Although the gurus at NYU say that there is no need to have retrievablity for implant crowns I agree with Rich that there are times that you wish they were! I now use poly-carb with a small drop of H2O. Sets faster and is pretty much retrievable while not popping off in function. Also as Rich mentioned, the parallel walls should give a decent mechanical fit. When I get really brave I'll particle abrade everything and cement with Rely-X. _________________ Gerry |

I also concur that it's usually preferable for an implant crown not to be permanently cemented. If the implant ever loosens, removal can be horrible.
Then again, I am wondering why this crown came off at all. It should not have because of the minimal taper. Just about anything should have retained it. _________________ "You need me on that wall."

Oh- perhaps avoid scaling the implant while cement is setting.

According to the instructors at NYJ you should have enough faith in the fit of the abutment and the proper torquing of the retaining screw to cement the crown with "permanent" cement. Nontheless I'll stick with something I can (hopefully) remove but the patient can't. As for the implant itself loosening - should this happened you're screwed anyway! _________________ Gerry |
