Hi everyone,
I've been having some problems with Surpass lately so I thought I come here to get some feedback.
I started using Surpass + IDS (about 2 month ago) for all vital teeth crown preparation after reading all the amazing stories of dentist using this technique and not needing to anesthetize 90%+ of the patients during the crown seat appointment. Much to my disappointment, the technique didn't work well in my hands. I was still getting a lot of sensitive patients and really didn't notice much of a difference between this technique and my old technique (which was applying Microprime G for 1 min after prepping and before cementing the temporary). So at delivery its hard to clean bits of temp cements, rinse and dry (like Dr. Kanca suggested) if I don't numb the patient.
This is the protocol that I use. After prepping the tooth,
(1) Pack cord
(2) Make temporary
(3) Surpass 1, agitate for 10 sec on the tooth (DON"T rinse)
(4) Surpass 2, apply generously and air dry
(5) Surpass 3, apply generously around the tooth, air dry really thin, cure for 10 sec
(6) Apply isopropyl alcohol to remove O2 inhibited layer
(7) Take impression
( Cement temp with TempBond NE
Am I not doing the IDS technique correctly? Should I leave a thicker layer of Surpass 3 on the tooth instead of air dry it really thin?? I know Surpass is a really good product and I really want it to work. |

Hi Dr. Kanca,
I'm not drying or rinsing Surpass 1, I just apply on the tooth for 10 sec. |

I do wash the prep with alcohol, but not until after surpass 1/2/3.
i make the temporary first, surpass 1/2/3, wash with alcohol, then take the impression.
do you suggest that i change the order of my protocol a bit? |

The temporary will leave goo on the prep that needs to be washed off before sealing it. Alcohol will do the trick nicely. _________________ No, Donny, these men are nihilists, there's nothing to be afraid of.... |

Consider washing the tooth with alcohol after making the temp. Temp fabrication does leave a residue. _________________ "You need me on that wall."

I sometimes have a problem with sensitivity at crown seat, but not very often.
My protocol is, take impression, make temp, seal with Sim, seat temp.
Reading the discussion, I'm wondering if I should clean with alcohol after making the temp and before sealing? Secondly, I have always used Sim to seal crown preps rather than Sur, since I felt that Sim left a thinner film on the tooth and there would be less chance of the seal interfering with seating the crown. Is it better to use Sur to seal? |

Nothing solves everything.
You might want to try Surpass for sealing preps and see if it makes a difference to you. I have been using it for a long time under preps. Just thin it out a lot and it'll be fine.
And yes, I recommend washing the prep with alcohol after making the temp and after the impression. Both procedures leave remnant monomer. _________________ "You need me on that wall."
